In 1921, Edward, Carlisle, and Dorothea have settled into a family dynamic.

Dorothea has started attending school with Edward, though they both remain standoffish and refuse to interact with the humans around them. The Cullen name, which they both took along, claiming to be Carlisle Cullen's children, is gaining a good reputation, and they are determined to maintain it.

While Dorothea still accompanies Carlisle to the hospital, she no longer works as a nurse. Instead, she acts as his "daughter" and spends her time in the waiting room reading magazines. It's during one of these waiting sessions that Dorothea picks up a familiar scent—blood, but not just any blood—it's something she hasn't smelled in years. This scent leads her to Esme, Carlisle's mate, a woman whose sorrowful past intrigues Dorothea.

Esme, at the age of 22, had married an abusive man to please her parents. However, after he was drafted into World War I, Esme experienced a brief period of freedom, until he returned in 1919. After discovering she was pregnant, Esme fled to her cousin's house but was soon hunted down by her husband.

Escaping once more, she went to Ashland, posing as a war widow. Tragically, Esme's child dies from lung fever in 1921, leading Esme to attempt suicide. Dorothea, sensing her pain, empathizes with her, much like she did for Edward's mother, Elizabeth. After feeling Esme's history and struggles, Dorothea decides to leave her room, advising Carlisle to visit her.

Upon returning home, Dorothea is shocked by the state of Edward. His once golden eyes are now ruby red, signaling his fall into the temptation of human blood. This causes a confrontation between Dorothea and Edward. She's horrified to see him so consumed by bloodlust, and she calls him out for turning into a "monster."

Their disagreement escalates, from pointing fingers and placing blames, into Dorothea using her power to control Edward, commanding him to step back. She warns him that if he continues down this path, he must leave the Cullen family. After a tense moment, Edward, unable to reconcile with Dorothea's words, packs his things and leaves the house.

"Edward Anthony Masen," she had called out to him, her voice low and commanding, the words laced with a dangerous authority. The same authority she had used to silence him in that moment of madness. "If you want to continue down this path of drinking human blood, then I suggest you leave. Leave now, and never return."

It had been the hardest thing she had ever done. To speak those words to the brother she had come to love as her own. But it had been necessary. The weight of her power, her foresight, made it clear to her that the future she saw for him was a grim one.

Edward had left, of course. His pride and his rage had driven him away, and in that moment, Dorothea had feared that he might never return. But she also knew that sometimes, people needed to walk away from everything to truly understand who they were meant to be.
