Makoto has a severe anxiety disorder and has frequent AND uncued panic attacks, though the only 3 people who're aware about this right now are Sayaka, Chihiro, and Byakuya.
Sayaka, Chihiro, and Byakuya all learned about Makoto's anxiety and panic attacks after Makoto had a bad anxiety attack during his, Sayaka, Chihiro, and Byakuya's monthly picnic in the playground the 4 always played in when they were kids and Sayaka, Chihiro, and Byakuya were, however, able to calm Makoto down from his anxiety attack.
A few days after the picnic incident, the 3 built a code that they would use in case if Makoto had a panic attack in the future, which indeed happened quite a bit!

But anyways, Sayaka, Chihiro, and Byakuya's protocol for when Makoto has a panic attack and is quite (Quote on quote) complicated, or at least that's what Toko states it to be.
It's also worth mentioning that Yasuhiro is also aware about Makoto's anxiety and beli it or not, he actually trained himself and knows EXACTLY what to do, should Makoto have a panic attack
