NSP Self Form: Self-Declaration of Minority Community Certificate is a document self-attested by the individual, stating that they belong to one of the minority communities recognized by the Government of India. As per section 2(c) of the National Commission for Minorities Act (1992), the following are verified as minority communities – Sikhs, Buddhists, Christians, Muslims, Parsis, and Jains. So this certificate is quite important if you need to access some government schemes and reservations (usually in the field of education, jobs, special schemes, and other related activities).

Also, the self-declaration of Community Certificate usually contains the name of the applicant, their father's name, and their residential address. The certificate also mentions the community to which the applicant belongs. Apart from the details of the applicant, other details like the date and place where the applicant made the declaration also recorded. Please note, in some cases supporting documents such as Community Certificate may also be required. Apart from this, the may also need to provide a copy of his ID proof.

In Hindi: अल्पसंख्यक समुदाय प्रमाणपत्र की स्व-घोषणा व्यक्ति द्वारा स्व-सत्यापित एक दस्तावेज है, जिसमें कहा गया है कि वे भारत सरकार द्वारा मान्यता प्राप्त अल्पसंख्यक समुदायों में से एक हैं। राष्ट्रीय अल्पसंख्यक आयोग अधिनियम (1992) की धारा 2(सी) के अनुसार, निम्नलिखित अल्पसंख्यक समुदायों के रूप में सत्यापित हैं – सिख, बौद्ध, ईसाई, मुस्लिम, पारसी और जैन। यह प्रमाण पत्र काफी महत्वपूर्ण है यदि आपको कुछ सरकारी योजनाओं और आरक्षणों (आमतौर पर शिक्षा, सरकारी नौकरियों, विशेष योजनाओं और अन्य संबंधित गतिविधियों के क्षेत्र में) का उपयोग करने की आवश्यकता है।
