Note- Sai still gets selective mutism after the accident in the story and doesn't get talking immediately after 1-2 sessions, as shown in the serial... mental health is not a joke.
Third POV
When the reality of the situation sinks into Sai, she finds herself unable to speak. But previously where she was mourning the loss of her father and traumatised by the treatment she had gotten in her new home. Her cries are for the loss of her children, the lonely future she lived and the fate she dealt with. Now that she has come to terms with the situation, she finds herself filled with deep loneliness again, as she knows she's in the past, but Virat doesn't remember, and her children are not born yet.
And when Pulkit Jiju brings a lady with him in her room. Sai already knows her, and her mind supplies her name, Dr. Anjali, even before she is introduced. It's not her imagination; her mind didn't make up these things, she has already lived all this, as everything happens mostly the same as before. But she doesn't speak.
The one-sided conversations somewhat go the same between the two, just the list of faces of her loved ones has grown. If someone assumes something different, then her silence answers their questions. She remembers her two lifelines, Vinayak and Savi.
She is diagnosed with depression and PTSD.
.
Doctors visit and constantly check her condition and progress. What another constant becomes is Virat, who is always there... holding her hand unless it's necessary to leave it. Sai keeps quite a grip on it during the day. Virat also doesn't question her need to hold his hand but is happy nonetheless to be given the opportunity to be there in her presence and be a source of comfort.
A Virat, who also wakes up at every flinch that Sai makes in her sleep. His sleeping hours are mixed up in the air due to his worry for Sai, but he also dreams of Sai berating him in her full sherni mood, but it all shatters when he finds in waking hours Sai listlessly staring out of the window, all silent. Virat yearn to hear her voice again.
But nightmares also frequently feature, making him fear to sleep, where sometimes he gets a call from the hospital to identify Sai's dead body. Sai was found dead even before reaching the hospital, Sai flatlining after having fatal seizures, or doctors announced Sai dead after an unsuccessful operation. Each leaving him rattled to the core.
He constantly finds peace and solace in hearing Sai's continued breathing, and his gentle feeling for her pulse brings him a profound realisation of how close he was to losing her to death this time, it brings new meaning to the importance of Sai in his life.

.
When Patralekha enters the hospital room, the only thing Sai wants to do is jump from her bed and kill her with the bouquet she brought, her injured self be damned. But controls herself, a few tears escape as Sai remembers the most hateful things Patralekha did to herself and her family.
That bitch had cursed Sai to miscarriage. Took her unborn child and blackmailed her into withholding her child from her. Brought so much division between Sai and Virat's relationship, causing the couple to rift apart and be separated for years and whatnot in later years.
.
Seeing Virat telling Patralekha off in the hospital room and making her leave the room. Sai sees a bit of hope for the future that Virat can take a stand against his so-called "best friend" Sai knows she can capitalise on that stance even though in years of marriage, Virat almost always took the side of his family first, even in their mistakes.
