Jay couldn't help but think about how much he keeps hurting Mac. He was having trouble letting her go. He knew he was already gone. Someone has to go and he knew that Mac would never leave him especially with all that he is going through so he was going to let her go no matter how much it hurts him. He knows they were always meant to say goodbye so he is going to do it for both of them.
