Hanahaki is a fictional disease in which a character coughs up flowers growing in their lungs due to unrequited love. It increases in pain as the character falls deeper in love with the other, and can lead to death. This trope is a physical manifestation of "love kills" when it isn't given back, or "cured."

Usually there's three options for the victim of Hanahaki.

Don't confess their feelings. By doing so, the victims will die but they don't risk the other outright rejecting them (which will secure their death)

Confess their feelings. This will go in two ways, the first is the other reciprocated the feelings and Hanahaki is cured. Or the other rejects their feelings and they die.

The surgery. Usually the surgery to be risky and have the effects of either losing all memory of the other or all memory of the other after they gain romantic feelings
