At age five, Miyako felt something in her rise to the surface. She didn't understand why she felt this strongly about it. Why did she feel the need to protect? It's not her job to protect, therefore she doesn't care. She continued to think this way until she eleven, when she felt something pulling on her heartstrings. She started feeling ill, coughing up blood at random and her breathing becoming abnormal. It got to the point her parents took her to the hospital.
She has an ability. Which was heartbreaking to her, for one reason in particular; her parents hated ability users. They ended up kicking her out and she made it her goal to find out who she was born to protect. It took her another five years to track him down, but she found him.
Ryunosuke Akutagawa.
Protecting him was easy enough, only a few thugs here and there. She got herself a job and began saving up to buy a house. She even adopted the Akutagawa siblings. She was happy with her new life. Insert a new problem by the name of Osamu Dazai.
"Can you give me any reason at all as to why I should live?"
"Yes, I certainly can."
Miyako followed him to the Port Mafia, bringing Gin with them. If Akutagawa would be happy in the Port Mafia then he can be in the Port Mafia. Even if she wasn't happy, his happiness was all that mattered. She needed to protect Akutagawa. She left her old job and Mori gave her a new one; being the doctor of the Port Mafia.
Miyako didn't like Dazai. He was abusing Akutagawa, but Akutagawa didn't see it as abuse. She couldn't stop Dazai because of that. There were a few times she had to step in, but that was it. She was thrilled, even if she didn't show it, when he left. She had to comfort Akutagawa for a while after he left.
She was content with her life.
