This is kinda like a what if more than a headcanon, but I LOVE Lord of the Rings so deal with it.
I think the real reason Frodo wanted to leave with the elves was because he had feelings for Sam. Like, he knew that Sam was in love with someone else and they never had a chance together, but he couldn't help but fall for him. And why wouldn't he? Sam was always there for Frodo, even when he volunteered to be the ring bearer. But Frodo never wanted come to terms with his feelings, so he avoided Sam as much as he could, even when he was his only friend with him on the journey to Mordor, like when Frodo tried to leave Sam behind on the river AND the Stairs of Insanity. But what was the breaking point for Frodo was when everyone came back and got on with their lives, including Sam. When he gets married and has children, Frodo is heartbroken because now will never have Sam and will live in regret for the rest of his life. That's why he left with the elves.
Bonus: It isn't the stab wound from the Nazgul's sword that keeps Frodo in pain, it's his aching heart that yearns for his lost love.
