hi friends!!

if you're here looking for me to make you a cover, welcome!! if you don't want a cover, thats okay too

this is the form you'll need to fill out for me to complete the cover. if you don't fill this out i won't make you a cover, because it's got the information i need to make it :-)

before you request, please keep in mind that i spend time on these covers and i put care into them. if you do request, please actually use the cover!! i dont wanna spend a long time working on a cover for you never to use it

anyways yeah happy requesting!!

2. title of the book and what you want as the author's name
3. quote (if you want one)
4. plot
5. style of cover*
6. how you want it delivered

side note: there is a separate form in each style chapter and it's up to you if you wanna use that form or this one :-) this ones just more general
