I do make requests, so this is the form to request a cover! I know I probably won't get many requests, but I'm doing this anyways.
For a cover, all I ask for is a mention somewhere in your book, like in the description or in a chapter.

Book Name: Just the name of your book!
Book Author: This can be your real name, your username, your pen name, or the name of the person you want to gift the cover to!
Actors: If you want to assign anyone as a character in your book, you can list any actors/images you want me to use and the roles they play in your book. A maximum of five individual characters to put on the cover (because it can get too crowded). If it's an anime character or image, or if the character is just a general hair color-eye color etc., I'll do my best to find an image that looks like your description.
Theme: Please state if your book is romantic, mystery, thriller, western, etc. You can list color schemes for me to use, as well, or a background for the cover.
Setting: Try to put the most used setting in the book, and the land around it (i.e. woods, lake, ocean, beach, town, city, etc.). This can be a common fictional place (like Hogwarts), a real place (like London), a general area (like a mountain range, or an unidentified small town), or a general fictional place (like an unidentified castle).
Sub-heading: Optional. You can include a sub-heading just to add extra text.
Fandom: Only if your book is a fanfiction. Otherwise, no fandom needs to be specified.
Short Summary: Please write a short summary of the book. This helps me get a feel for the genre we're working with, and what styles I should use.
