Hey, whassup my unicorns!! I've decided to start a small book dedicated to creating book covers for those who need them! 

Rules:

1. You may request up to 5 books. If I have already created 5 book covers for you, then PM me and I will see if I can work something out. This is the only time you can PM me. Please don't PM me otherwise.

2. Don't be rude or inconsiderate -- I'm not a professional cover maker, so they're not going to be magical, disney-worthy pieces of art, but I'll give it my best shot.

3. I only create covers for stories that have been published, so if it hasn't been published, don't waste your time here. It doesn't have to be completed, it can be in progress, but it must be published.

4. Please complete the form below with as much detail as possible, so I can make you the best cover possible. Also, please complete the form all in one comment.

So that's the entire form! Make sure to fill out as thoroughly as you can. If I accept your story, then I will reply on your comment with "Request accepted". 

Here's the payment for having a cover created:

1. Follow me.

2. Vote on at least two chapters of one of my books.

3. Give me credit in the summary of your book (this can be done after the cover has been created and uploaded to your story).

If you do not complete the payment then your cover will not be created.
