Naruto had cancer in his aortic valve and had a complete heart transplant from which he got his heart from Kakashi and a blood transfusion from Iruka - his legal guardian.
Tsunade is Naruto's primary doctor and Shizune is Sasuke's. Both have Mebuki (Sakura's mother) as their primary nurse, thus they learn about each other at their regular hospital visits.
Naruto has to take medicine for chronic anemia due to the blood transfusions and uses a heart monitor and pace maker to assure he's not gonna drop dead (otherwise known as his heartbeat doesn't increase to a point his body can't handle as Kakashi was much more active than Naruto is and could handle much more).
Sakura is Naruto's childhood friend through Mebuki as she doesn't really have any other people to talk to - afraid to get made fun of for her appearance (in which Naruto usual shows her his scar for comfort and they both end up smiling and doing friend things).
That's basically it that changes, but the concepts the same.
