NURSE’S NOTES

A/N: Ano ba ang nurse’s note? Ito ay ang notes na pinagsusulat ng mga nurses sa hospital. Kadalasang sinusulat nila ang lahat ng nangyayari at ginagawa nila sa isang pasyente... it is their form of communicating with the other health members na tumitingin sa pasyente. Sa pamamagitan nito malalaman nila agad kung anong naibigay na mga gamot o anong mga ginawang paggamot sa pasyente.
ER Notes
7-3 shift
> Came in to the ER this 18-year old, beautiful, female via stretcher; conscious and coherent
> She is wearing a brown leather jacket and jeans torn out because of the accident; she’s drunk and is crying from pain
> She’s the most beautiful and charming creature I’ve ever seen and for a moment I thought the world stopped from moving as I gazed upon her
> Vital signs taken and recorder; Her name is Rica and she is my first patient of the day
> She had multiple bruises and abrasions on both lower extremities; burn wounds are also seen as we cut some portion of her jeans to expose the wounded area
> I get to hold her hands as she screams from pain; Nurse Judy cleansed the wound while I tried to insert an intravenous fluid line on her left arm
> She screamed and got me out balanced as she holds on to me; finally I’ve managed to calm her down and inserted a patent IV line for medications
> “Please don’t leave!” she verbalized and holds my hand tightly
> Doctor Sanchez and nurse Judy performed a minor surgical operation on her while I stayed beside her
> It’s not part of my job yet I can’t move away from her; she’s crying, tears falling from her eyes; I want to comfort her and yet holding her hands is all I can do
> Suturing was done; Doctor Sanchez ordered medications for her while Nurse Judy cleansed the instruments; I’m left with her to continue wound dressing and bandaging
> Laboratory examinations were done; she is afraid of needles and blood but I was still there so that she will feel she’s not alone
> I also accompanied her to the radiologist to check her legs; she had an x-ray examination done on her leg because it’s swelling
> “Thanks,” she whispered after all the examinations on her was done
> I just smiled at her but instead she turned away’ There was an awkward silence between us as I moved her back to the the ER for further observations
> I constantly check on her; IV fluid is Infusing well on her left arm regulated at 20 drops/min; her vitals signs were also within normal limits
> I don’t know why she can’t look at me in the eyes; I feel sad yet I continued to smile to cheer her up
> “No fractures or broken bones,” I said to her as the results of the x-ray examination came back; I saw her smile and again the world just stops from revolving; I want to see her smile everyday; I want to make her smile but how?
> My shift is about to end and she’s stable so I have to transport her to the ward; I know from then on that I’ll miss her; I just smiled as we soon part ways
> Endorsed to ward; My heart was still with her

Nurse on Duty
Addendum:
I’m off duty tomorrow... Maybe my saddest day off ever. Shall I visit her? I know it’s against the rules but I am not sure if she will still be there when I come back. I sneak out a visit to the ward just to look at her and she saw me. I smiled but she just turned away. My heart, it was shattered. I left and never went back until my next duty.
