Idea of the story is that a izuku midoriya gets a power or quirk every time he dies.
Izuku midoriya quirk is when he gets into a situation that would usually end in death his Quirk turns that situation into a power corresponding to the to the situation which he died.
The quirk wasn't active for most of his life because the last time he used it was when he was born.He was originally born stillborn because of this his quirk activated prematurely and he didn't get any powers from the death( but even prematurely the base of the quirk is not to let him die ) but he's had his quirk since birth. It fully matured when he reached the age of four like all the other kids. But since he hadn't died since birth he did not get any powers to prove it. So he was thought quirkless not only that but will not activated the quirk is almost undetectable.
