The gash on his mouth was not caused by the car crash. Since he was numb he was able to chew through the skin to relieve his stress. He also had a habit to chew anything that he could grab a hold of if he didn't feel like chewing on himself. But mostly he didn't want to leave his room so that was one more big problem
