He knows that if he continues seeing her, his home life and relationship with his father is only going to get even worse than it already is, but without her, he just keeps spiralling down further and further. He just needed to get his mind around everything that has been going on. Knowing that there's not a lot he can do about not being able to see Juliet, Andy is facing one of the worst decisions of his life, and if he chooses wrong, it could result in losing someone he can't live without.
