Izuku was sick his whole life. The doctors didn't know what he had, just that he would propably die when he was really young. Maybe not even an adult. He spend his whole life in the hospital and was only a few times outside in the forest or the beach. He was never in a mall or in the middle of a city (except if he is in the hospital), because the chance that he infects himself with other sicknesses was to high.
