The womb is there for a certain period of their life time, between the ages of fourteen and their late thirties. Being a carrier is just much like a women. Their womb is much like a uterus but without the whole period process, the reason Keith looks bloated it's because the womb is already at its full growth and ready to have a baby. Keith does not only produces sperm but produces eggs as well, they are disposed through bowl movements. So he doesn't have a bloody process as women do. There will be some complications throughout the pregnancy like not being able to produce breast milk or actually being able to have a natural birth. Being a carrier is uncommon, only a handful of men have this womb. Keith happens to be part of that handful.
