It just means that when someone is faced with torture or something similar to it you will gain a quirk. It's pretty rare so he doesn't use it much. This quirk allows the owner to have a certain thing happen to him based on his emotions. Deku had gone through a Femboy phase so he just goes with it now, no matter what people say or think and somehow got a harem along the way.
