It's been years since then. One day the Straw Hats had gotten into a bad storm. Sanji had fell over board because of the strong waves rocking the ship. Zoro dived in to get him. Luffy pulled Zoro back onto the ship once he had gotten the unconscious cook. When they got him back on the ship he wasn't breathing. They freaked out when they checked his pulse; he didn't have one. The cook disturbs the swordsman on watch because he can't sleep. He needs his bolster - it is big and warm and fluffy.
