Kaori Miyazono has no way out. The symptoms of her illness have worsened, and the only way out is a dangerous surgery that could also easily kill her if something goes wrong. Her doctor, a mysterious but kindhearted man, takes pity on her and offers her an alternative: a simple blood transfusion may be all she needs to alleviate her illness and become strong enough to play another duet with the boy she loves, Kousei Arima. And he himself will be her donor. Kaori and her parents accept this offer, grateful for the doctor's help. But they have no idea of the hell they are about to unleash. For this doctor is the greatest villain from Japan's Taisho Period, thought defeated at the hands of the Demon Slayer Corps. The Demon King...Muzan Kibutsuji.

In the end, Arima Kosei was saved from his fall but his beloved, Kaori, died. But what if there is an Alternate Universe wherein Kaori Miyazono did not die? What if Kaori Miyazono does not have any unknown incurable illness in the first place? After the last piano recital of Kosei, where Kaori was supposed to die, everything went back to normal. Tsubaki was still looking over Arima and continue being a softball player, Watari decided to move on from his one-sided love with Kaori and focused on soccer club. Arima and Kaori are still friends, sometimes going out to eat Canelé, playing in the river or in the music room. Everything is normal yet perfect. The time continues to move. Except for Miyazono Kaori. The brave and lovely Kaori was in the dark pit. She was drowning yet no one can see. This is the parallel world wherein Kaori did not die because she was sick since she was a kid. But this is the parallel world where Kaori wish nothing but death. ****TRIGGER WARNING! An alternative ending to Shigatsu Wa Kimi no Uso (Your Lie in April).

Like other cliché story about what if kaori survived the surgery,this story will go on the same direction.But as we know,her operation only prolonged her lifespan not cure the disease.But at least it'll give some time before the cure hopefully discovered.How Kousei will react that Kaori liked him and want to date him but knowing the fact that she probably will have short time to be with him?How will Tsubaki and Watari react to it?

Neither of them understands love yet and experiences the growing pains of it. A sad story about not wanting to hurt the other, thus distancing themselves.
