Hanahaki is a disease developed by the suffering that is unrequited love. It causes flowers to grow in a persons airways, forcing the victim to cough them up until they completely fill their lungs, killing them. All this is found out when Akutagawa starts researching his constant flowery coughing. He isn't surprised by the cause, after all, he could feel the smile creeping on his face when he looked at Atsushi.
