In the first few months of UA, he always avoided the hero course classes because they'd be able to tell something was wrong with him. Long story short, Izuku had a drug addiction. It wasn't to one specific thing like Adderall, no, it was much more than that. He knew the dangers of it, but, he didn't care. What'll happen when Class 1-A starts to notice him?

It's getting sad at this point, but I'm still having too much fun to stop. I really wanted to tap into Pierrot's mind in this one, because as much as we all love him, let's face it, he's crazy.
