He was killed by Meetra Surik and her small team. Dead, and all that remained was a haunting mask, long lost to time. Or so everyone thought. This is a combo of the Old Republic and the New Jedi Order. Also lots of non-canon references thrown in there, such as Force Unleashed and Knights of the Old Republic.
