Une heure plus tard, le patient qui venait d'être déclaré mort bondit de son lit pour attaquer sauvagement une infirmière. Dans sa folie meurtrière, Chung avait tué deux personnes et en avait blessé trois autres, avant d'être finalement abattu par un agent de sécurité. Au cours de la nuit, ceux qui avaient été mordus tombèrent malade à leur tour et attaquèrent les autres. L'infection se propagea très rapidement. Mal préparées, les autorités essayèrent de contenir les infectés avec l'escouade anti-émeute, mais ils s'aperçurent bien vite que les bâtons et les gaz lacrymogènes n'étaient d'aucune utilité contre des morts-vivants. Le virus se propagea rapidement parmi ceux qui n'avaient pas eu le temps d'évacuer la ville. Confronté à un désastre d'ordre international, le pays fut mis en quarantaine, mais il était déjà trop tard, rien ne pouvait empêcher les infectés de traverser les frontières. Le virus avait été amené sur les continents par des voyageurs contaminés avant la fermeture des aéroports. En raison de leurs fortes populations, les grandes agglomérations tombèrent rapidement aux mains des infectés, alors que les plus petites villes et villages résistèrent un peu plus longtemps, sans pour autant échapper au virus. Central city a attiré de nombreux survivants le jour où les militaires ont décidé de s'y installer.
