He avoids hanging out with loved ones and shuts out anyone who tries to help. Ochako is pained seeing him like this but that's not the only pain she feels, soon she develops the Hanahaki disease, a disease caused by one sided love. It's not that Deku doesn't care for her it's that he can't care for anyone, all he does is obsess over the ways he could have saved him....
