When he was 4, Izuku was deemed Quirkless. Having two joints in your pinky toe was a sure way to prove if one was born with a Quirk or not. There were only ever 2 cases in all of documented Quirk History where someone with the extra joint had developed a Quirk, so it was with absolute 100% certainty that the doctor had informed the crushed child and devastated mother that he would never develop one.
