Set inside an alternate version of the The Outlast Trials universe, the story follows the growing relationship between the mysterious Ms. Miller and Franco Barbi within the Sinyala Facility. While reagents struggle to survive Murkoff's experiments, rumors begin spreading about a woman in a porcelain mask wandering freely through restricted sectors despite having no records, no known history, and no official connection to the Trials. Calm, soft-spoken, and disturbingly polite, Ms. Miller blends herself among reagents and staff alike, earning trust moments before turning violent without warning. As Murkoff investigates her unexplained presence, it becomes increasingly clear that her attachment to Franco Barbi is far from normal. Rather than fearing him like everyone else inside Sinyala, Ms. Miller seems drawn to his brutality, quietly observing his broadcasts, executions, and outbursts with unsettling fascination. In return, Barbi becomes obsessed with her attention, treating her less like an intruder and more like someone who belongs beside him.
