Since the most evil and despicable acts are done consciously, there's a catch: the darkness within takes form only when the person is conscious, therefore, the patient must be fully aware and awake during the surgery. Not the pubblic nor the the Print or even the government knows what happens after this darkness is removed. It does not help the fact that the surgeons categorically refuse to explain what happens before, during and after the operation, not even saying what or how a person looks like once everything is done. Some of them even denying having performed said surgery.
