Sakura never said anything about him having a friend or not. He figured not so long especially after Kotoha's visit when he was sick, or the way everyone seemed to include him in basically everything they did, or he might or might not have been eavesdropping some conversations where everyone assumed he was lonely from the start and had no one to lean to.
