He was killed by Meetra Surik and her small team. Dead, and all that remained was a haunting mask, long lost to time. Or so everyone thought. This is a combo of the Old Republic and the New Jedi Order. Also lots of non-canon references thrown in there, such as Force Unleashed and Knights of the Old Republic.

When your father, a former First Order trooper, is killed by the Resistance in front of you, your feelings about them change dramatically. Full of fury and hatred for the Resistance, you join the First Order and become someone the Resistance fears.
