Pure Vanilla, now known as Truthless Recluse, is a mentally ill patient at an mental hospital, usually harming or threatening his doctors due to trust issues and other problems. But he soon meets Shadow Milk, also known as Sage of Truth, trying to help him heal and find peace. Truthless Recluse hated this, he just wouldn't go away despite everything he does. Now Truthless Recluse is left with these feelings and emotions, not sure what to think of his new doctor.
