After being forced to give up he and Rose's daughter, facing the army of ghosts, losing Rose at Canary Wharf soon after, saving Donna, and struggling to carry on, the Doctor wanders off again. Mostly to regain his sanity. There's really nothing out there for him except the thought of finding someone like Donna said. He didn't exactly take her advice, but improvised on it. And that's just what led him to meet two certain people; for better, or for worse.
First, he meets Eileen Cipher. A young, typical, Scottish girl who he wouldn't know anything about for a long time. Second, he meets Martha Jones; a medical student in London like no other. Time and curiosity leads these three people to meet all at once for a sudden trip they would have never expected. It took some Judoon, and some self-convincing, but the Doctor realizes that these two girls are the people that Donna was talking about, and he takes them along.
What none of them know, is that all of them are holding onto their states of mind like a rope. Eileen's rope is the state of mind of what she is or isn't. The Doctor's rope is of loneliness or company. Martha's rope is of captivity or freedom. The mysterious Mr. Saxon is carefully cutting their ropes, and if they let go...
But until then, the three know that the universe awaits to throw things at them left and right, whether they like it or not. To them, that's just the way they like it.
