The idiot is Shinya because this is Guren's POV but in fact Guren's the idiot. Shinya got injured and turns out he was feeling ill the whole time, Guren the most worried "Friend" was the one who worries the most. Or Shinya falls sick and Guren takes care of him, Eventual confession. (I posted this fic first at Ao3, So if anyone from AO3 is seeing this, I'm just reposting it here.)
