Alexa Flanel, or Alex to her friends was living peacefully in America until she got a scholarship to Ouran Academy. Alex then goes to live with her Uncle Ranka and cousin Haruhi. She ends up going and just like Haruhi, everyone thinks she is a boy. Even the Host Club who figured out Haruhi was a girl doesn't figure it out. This takes place after the shows first season. Some events are kind like the first season.

Haruhi was only just coming to terms with her feelings for him, and so it was hard for her to do even slightly romantic things with him at all. It always made her shiver whenever Tamaki leaned in to kiss her, so much so to the point where they've never had their first kiss with each other.
