Thomas is with Anxiety in the mindspace. A city inside Thomas's brain. It links his brain to all sorts of places and it's how the personalities and emotions get food and other things that are essential for any being's existence. Thomas forced Anxiety to take him into the mindspace and they end up eating ice-cream from a quaint little ice-cream parlor. On their way out of the mindspace, Thomas somehow walks in front of an approaching van. What will happen to him? What will happen to Anxiety and how will the other emotions react?
