This is the story about Kyoya and his twin sister. She appears at the doors of the host club and starts a fight with honey and mori before Tamaki and Kyoya get there. This happens at the ending of the 3rd episode when they go back to the club room to find mori and honey fighting a girl.
The girl on the cover is Kyoya's sister except she has black hair and gray eyes.
