Blaine had hurt Kurt. Not physically, but mentally and it was increasingly hard on his heart. His now shattered heart.
And Blaine notably feels guilty of what he'd done to Kurt's heart knowing, that it was all because of him and no one else.
But, a year later after their 'break up' they meet again as Blaine was auditioning for a part in the same musical as Kurt, had already auditioned for.
