Tsuna is reborn! (No, not Reborn/Renato)

Killed by a disease, Tsuna found himself in another world were youkai are everywhere and a certain clan is waiting for him.
He gains a new family but what happened between him and his parallel-self real family.
Will he never see his famiglia again? Or he will forget about them?
