The main character of this story frequently calls people brother or sister. None of the people in which he mentions this way are his brother and/or sister. 
This story is also heavily based of the film "A Clockwork Orange", which was adapted from a book of the same name. If you haven't already, I would suggest read the book or watching the film, for they are both amazing.
