Gumball is being bullied (Yes also by Marshall)- which is one of the reasons why he is depressed. He is really emo for which he gets bullied even more. 

Marshall is a fuckboy. He is the coolest guy in school and fucks literally everyone who is not on the tree untill three. Yes. Even some teachers.
He bullies Gumball and other people of course but as soon as he found out about Gumball's secret, he started to be nicer.

Gumball had asked him on multiple occasions what was wrong, but all he got in response was "it hurts" and after Gumball comes and sits next to Marshal, he hears marshal saying that the pulsing got faster. Asking where it was, and finding out it was his heart, he is shocked. Etc etc etc.
