Akcja ma miejsce w przyszłości, w samowystarczalnej Japonii, odciętej od reszty świata, kierowanej przez Sybill - system komputerowy potrafiący przeskanować człowieka, analizując jego talenty, słabości, a nawet skłonność do popełnienia przestępstwa. Główna bohaterka, Roa Onoue Ren, po ukończeniu szkoły i zdaniu egzaminów z najwyższą możliwą notą zostaje przydzielona do specjalnej jednostki policji zajmującej się prewencją - eksterminacją ludzi, których psycho-pass przekroczył bezpieczną granicę. Wymaga się od niej bezwzględnego posłuszeństwa i żelaznych ideałów, na których szczycie oczywiście musi stać stworzony przez Sybill porządek, w którym ludzi dzieli się na tych z czystym i tych z zamglonym pass'em. Pierwsi mają dostęp do wszystkich dóbr, rozrywek i praw, natomiast drudzy, nawet jeśli nigdy jeszcze nie popełnili żadnego przestępstwa, skazani zostają na ośrodek resocjalizacyjny lub śmierć - w zależności od osądu panującego Sybill.
