When Joker escapes Arkham once again, he collapses in Bruce's arms. Bruce knows that something is wrong so instead of taking him back to Arkham, he takes the sick man to the Batcave. It's obvious that Joker wants any kind of attention from Batman. He constantly flirts and makes flirtatious jokes. But, when Bruce Wayne starts to have feelings for the clown, he ignores them, burying them deep down. He pretends that the Joker is just playing. He pretends that Joker is just messing with his mind. But he knows that they both have feelings for one another.
