Peter deals with a lot during his time as Spiderman, getting hit just seems to be one of those things. Something goes bad this time and his dads find him bloodied in his bed, passed out, in his Spiderman suit.
Peter also deals with a lot during his school life and home life, he gives himself shit for being depressed (even though he knows he can't help it), putting himself down because he's not what his dads singed up for when they adopted him. He knew it was wrong since the day he started when he was 8, but never found his way out from the harm he was doing to himself. He held back on cutting and let bad guys hit him but maybe getting hit by bad guys did more harm.
