Dean was just following Sam on another case. His visions had started again, and as far as Dean knew, that meant that Azazel was back. Dean had already lost his brother once to the Yellow Eyed Demon and he was not going to let it happen again. But as Dean chases his brother's vision, the two boys come across not a demon, but a girl.
