The emotional tug-o-war between giving in and fighting for all those that are counting on him to live. Even with the support of his friends and family, the story will center around Sans' perspective and the tension that arises in his soul.
Being sick is hard on anyone, especially those who are very intimately familiar with death and the feeling of it. It would be easier to die, but as they say;
