Kuchisake-onna (口裂け女, wanita bermulut robek) adalah sejenis siluman dalam mitologi dan legenda urban Jepang. Ia berwujud seorang wanita yang bermulut robek. Dalam legenda urban Jepang, ia menutupi mulutnya dengan masker operasi dan sering muncul di jalan-jalan yang sepi. Ia bertanya pada orang yang ditemui apakah dirinya cantik. Bila orang itu menjawab tidak atau ketakutan melihat wujud seramnya maka ia akan membunuh orang itu. Bahkan pada akhir 1970-an, beberapa sekolah menyarankan agar murid-muridnya pulang secara berkelompok atau ditemani guru agar selamat.
