Un protagonista anónimo encuentra un día un diario en un hospital que pertenece a su compañera de clase Sakura. En él descubre que la chica padece una enfermedad terminal en el páncreas y que le quedan pocos meses de vida. Sakura explica que el protagonista es la única persona ajena a su familia que conoce su condición y este le promete guardar su secreto. A pesar de que sus personalidades son totalmente opuestas, el protagonista decide pasar junto a Sakura sus últimos meses de vida.
