it's used to refer to the intellectual thought to engage in destructive behaviours 

example: feeling the urge to jump off a cliff edge while standing on it 

thinking about swerving into the opposite lane whilst driving

seth fears his life the day he almost drowns himself simply because he has the urge to, fears himself when he nearly places his hand in a fire because he he feels like it, he admits himself into a nuthouse
