Shigaraki Tomura, formerly known as Shimura Tenko wasn't always the cold blooded villain he is today. In the past where he was still young and innocent, he dearly loved his sickly younger sister. But after that fateful incident, he vowed that he will protect and take care of her; the best that his 10 year old self could. 

AFO and the Professor came and 'helped' the siblings, but Shigaraki's happiness did not last long. His sister was inside what the professor would call the 'Regeneration Chamber' but just when his sister would finally be cured from her sickness and be healthy, heroes appeared. Even when he blames you for how he ended up, you can't stop yourself from going back because of that sympathy, because of that pity.
